Who is the Mental Health Pathway for?​

The pathway aims to support mental health service users to submit clinical outcome measures digitally, which would enable clinicians to monitor patient treatment and further recovery.

How will it work?

1. The local mental health team adds the patient to the pathway.

What is this service?

The Mental Health service supports you to easily send information to your health professional (clinician, nurse, or other) from wherever you are, saving time in face-to-face appointments while helping you have your own view how care is progressing.

What do I need so I can use the Mental Health service?

What will I need?
In order to use the Connect Me Prostate Cancer service, you will need access to:
•A smartphone, tablet, laptop or PC
•An email address and (optional) mobile number
•A broadband or WiFi internet connection
