Who is the Heart Failure Service for? ​

This service is being clinically designed and developed in support of Heart Failure patients, specifically aimed for:

  • Newly Diagnosed Heart Failure Patients​
  • Patients with deteriorating/unstable Heart Failure symptoms

How will it work?

  1. The service requires patients to submit vital signs readings, including: Blood Pressure, Heart Rate and Body Weight (with the option to include Sp02), and answer a series of questions related to their health. ​
  2. Additionally a validated and scored quality of life questionnaire [Kansas City Cardiomyopathy Questionnaire -12 (KCCQ-12)] will be completed, with a message being displayed advising the patient of their score and how it compared to their previous scores. ​
  3. A third component will cover a psychological well-being questionnaire (PHQ-4). Following completion of this, a message will be displayed advising the patient of their score and how it compared to previous scores.​
  4. It has been clinically designed whereby dependant on the answers/ scores then the HF Nurses/or Clinicians will be notified with Priority or Urgent alerts but the HF Nurse may not have the opportunity to review immediately.

Objectives & Outcomes​

Remote monitoring of Heart Failure (HF) patients by specialist HF Nurses (or HF clinicians where applicable) to enable:

  • Remote optimisation and earlier achievement of guideline directed HF therapy, as tolerated, for patients newly diagnosed with HF, in order to reduce their risk of hospitalisation and premature death. ​
  • Closer monitoring and earlier intervention to improve quality of life and reduce the risk of hospitalisation, where possible, for patients identified with worsening symptoms and quality of life.
