What will I need?
In order to use the Connect Me Lymphoedema Questionnaire service, you will need access to:
•A smartphone, tablet, laptop or PC
•An email address and (optional) mobile number
•A broadband or WiFi internet connection
How do I use the service?
•You will receive an email with a link to the online health questionnaire
•Enter your date of birth to access the questionnaire
• Answer the questions about your swelling before your outpatient appointment
What questions will the service ask?
You will be asked to complete an online health questionnaire with information about your swelling. This will inform the Lymphoedema team and support your outpatient appointment. It will make more time in the appointment to talk about what matters most to you.
At the end of the service, you will receive links via email to further information and guidance about your swelling, while you are waiting for your appointment.
Six months after your appointment date, you will be asked to complete an online follow-up questionnaire.
What does the service do with my results?
Your healthcare professional will review your completed questionnaire before your appointment and will assist with any questions you were unable to answer.
A document of your answers will be sent to you via email.
What else should I know?
We hope you find the Connect Me Lymphoedema Questionnaire service enjoyable to use and that it will help you to understand and manage your symptoms.
However, if you choose not to complete the questionnaire, your care will be handled as it would have been before the questionnaire was being used.
