COPD

Who is the COPD Service for?​

This service has been clinically designed to support the following patients:​

  • A primary diagnosis of COPD ​
  • Moderate to severe disease (GOLD grade 2-4) ​
  • Increased use of healthcare in the past 12 months ​
  • One or more exacerbation of their COPD in the past 12 months, or be at risk of exacerbations ​and/or ​
  • One or more hospital admission in the past 12 months related to COPD  

How does it work?

1. The service has initially been built using only Mobile App and Web Portal channels in which the patient receives two sets of monitoring questionnaires:​

  • A COPD Assessment Test (CAT) to be completed the day after enrolment and monthly thereafter
  • A symptoms questionnaire, usually to be completed daily (including questions about overall wellbeing, breathlessness, cough, sputum, chest tightness and duration of symptoms)​
  • Optionally, a daily request for SpO2 readings can be configured​

2. The patient will then receive appropriate advice, based on the CHSS traffic light system, depending on their responses to the questionnaires​

Objectives & Outcomes​

  • The aim of the pathway is to reduce unscheduled care admissions and increase service capacity through early identification of those most ‘at risk’ of exacerbations and hospital admissions.  ​

  • The pathway provides regular tips for patients, plus appropriate education and advice depending on the patient’s responses to symptoms questions. ​

  • In addition, the pathway may be suitable to promote early supported discharge.
