National COVID-19 Self-Management Service

Who is the National COVID-19 Service for? ​

The COVID-19 Remote Health Pathway has been developed to support self-management of patients diagnosed with COVID-19: either clinically OR positive test result AND symptomatic.​ An additional service has been developed and implemented for use within Maternity Services.

How does it work?

It uses an updated version of the existing COVID-19 remote monitoring pathway, as a supportive self- management tool, with the addition of:​

  •  An easy-to-use registration service delivered by National Contact Centre. This will provide central administrative resource to onboard and provide support to people using the remote monitoring service​

  •  A supply of pulse oximeters, delivered to appropriate distribution points. This will include a patient information leaflet outlining the service and asking the person to register via the 0800 number provided​

  •  Further potential to connect remote monitoring with other developing community facing services such as virtual wards, intermediate care and provision of antiviral therapy

Objectives & Outcomes​

  • Improves early detection and intervention around the management and support of  COVID-19 patients ​

  • Improve patient’s confidence with technology and self-management of condition​

  • Reduction in Covid-19 healthcare impacts (better support out of hospital)​

  • Underpins and supports the Scottish Government’s Protecting and Renewing programme for citizens within Scotland ​

  • Enables effective use of technology in the triage and navigation of our healthcare services ​

  • Promotes self-management and a person-centred approach in effectively supporting and managing those citizens who have been diagnosed with COVID-19
